Dr. Rafizadeh's Lip Lift

The lip lift — also known as a subnasal lip lift or philtrum reduction — shortens the distance between the base of the nose and the upper lip (the philtrum), elevating the lip into a more youthful position, exposing more of the upper teeth, and creating a fuller, more defined Cupid's bow. Unlike lip fillers, the results are permanent.

Since 2012, Dr. Rafizadeh has performed lip lifts as a standalone procedure on a regular basis — at a time when most plastic surgeons were unfamiliar with the benefits of this operation. Today it is one of his most-requested procedures and his declared super-specialty. He has developed a signature technique for hiding the scars within the nostrils, making them less visible than the standard placement at the base of the nose.

“You are a very good candidate for a lip lift. I have developed a method of hiding the scars in the nostrils. I find this is the best way to hide them in an area that is less visible. In general the scarring is good and the incision heals very well. Over hundreds of cases I have only seen a few patients with slight hypertrophic scars that required a steroid injection.”

— Dr. Farhad Rafizadeh, MD FACS

The Golden Ratio & Lip Proportion

The ideal philtrum length is 11–13 mm for women and 13–15 mm for men. When this distance exceeds these proportions — either naturally or through age-related elongation — the upper lip appears flat, the teeth show less when smiling, and the face loses the youthful balance associated with the golden ratio. Dr. Rafizadeh measures precisely before marking the incision, and the patient must agree with the proposed changes before surgery begins.

Who is a Candidate?

Ideal candidates include patients who have a philtrum longer than 13 mm, diminished upper tooth show at rest, a thin or flat upper lip, or loss of Cupid's bow definition. Both younger patients seeking a more defined lip and older patients experiencing age-related elongation are appropriate candidates. Dr. Rafizadeh also performs lip lifts on male patients and finds that men generally heal very well due to the robust blood supply of the perioral area.

Lip Lift vs. Lip Fillers

Lip fillers add volume to the lip but cannot reliably shorten a long philtrum or increase central tooth show at rest. A surgical lip lift addresses length and dental display directly and permanently. Many patients continue to use small amounts of filler after a lip lift for fine contouring — the two are complementary.

Recovery Timeline

Days 1–5: Swelling and mild tightness. Be cautious with mouth movements. Cold compresses help.
Week 1–2: Sutures removed. Avoid full smiling and strenuous activity for additional healing time.
Weeks 2–4: Swelling resolves. The result is visible. Most patients can return to all activities.
Months 2–6: Scar continues to fade. Sunscreen daily is recommended.

Lip Lift FAQs

What is a lip lift and how does it differ from lip fillers?+

A lip lift is a surgical procedure that removes a small strip of skin from beneath the nose to shorten the philtrum (the distance between the nose and upper lip), permanently elevating the lip into a more youthful position and increasing the visibility of the upper teeth and the pink portion of the upper lip. Lip fillers add volume to the lip but cannot reliably shorten a long philtrum or increase central tooth show at rest. A lip lift creates structural change; fillers provide volume. Many patients use small amounts of filler after a lip lift for fine contouring — the two are complementary.

How does Dr. Rafizadeh hide the lip lift scar?+

Dr. Rafizadeh has developed a signature technique for hiding the lip lift incision within the nostrils — placing part of the scar inside the nostril opening rather than at the most visible base-of-nose location. He finds this is the best way to conceal the incision in an area that is less visible. In general the scarring heals well, and over hundreds of cases he has only seen a few patients with slight hypertrophic scars that required a steroid injection. — Dr. Rafizadeh, RealSelf

Am I a candidate for a lip lift if I am younger?+

Yes. While lip lifts were historically recommended for patients 55 and older, refined techniques now make them appropriate for patients as young as 25–30 who simply have a longer-than-ideal philtrum or diminished tooth show. Dr. Rafizadeh evaluates each patient individually based on philtrum length, vermilion show, dental display, and overall facial proportions — not age alone.

Can men get a lip lift?+

Yes. Dr. Rafizadeh performs lip lifts on male patients and finds that men generally heal very well due to the robust blood supply of the perioral area. The ideal philtrum length for men is 13–15 mm. Male candidates who have diminished tooth show, a flat upper lip, or a philtrum beyond 15 mm can achieve excellent, natural-looking results. Techniques for male patients differ slightly from female patients to preserve the natural masculine appearance of the mouth. — Dr. Rafizadeh, RealSelf

What is the recovery after a lip lift?+

Days 1–5: Mild swelling and tightness around the mouth. Be careful with mouth movements — avoid wide smiling and opening the mouth fully. Cold compresses help with swelling. Week 1–2: Sutures are removed. Avoid strenuous activity. Weeks 2–4: Swelling resolves and the result is clearly visible. Most patients return to all activities. Months 2–6: The scar continues to fade. Daily sunscreen is recommended to minimize pigmentation changes.

Is the lip lift result permanent?+

Yes — the structural change created by a lip lift is permanent. The philtrum is physically shortened and does not re-elongate. However, the face continues to age naturally after surgery, and the upper lip may lengthen again gradually over many years due to ongoing aging. Most patients are satisfied with their results for many years, and some choose a minor revision a decade or more later if desired.

No One Could Tell I Had Surgery

Dr. Rafizadeh gave me the perfect subtle change to my face that has made me feel beautiful and confident. My friend came over and had no clue I had anything done. She said I looked great — was it my skin? I can barely even notice the scar at two weeks post-op.
