Gynecomastia Surgery

Gynecomastia — enlargement of the male breast — is far more common than most men realize, affecting an estimated 40 to 60 percent of men at some point in life. For many it is a lifelong source of self-consciousness: a fullness of the chest that does not respond to diet, exercise, or weight loss because it is driven by firm glandular tissue, not just fat. Gynecomastia is now the second most common cosmetic surgery among men in the United States, and demand has continued to rise as more men seek correction. Dr. Rafizadeh has treated male chest contour for more than 40 years in Morristown, NJ, with a discreet, results-focused approach.

“Gynecomastia is rarely just fat. There is almost always a firm button of glandular tissue under the areola that liposuction alone cannot remove. I combine liposuction to debulk and feather the fatty component with direct excision of the gland through a small incision hidden at the edge of the areola. That combination is what gives a truly flat, firm, masculine chest.”

What Causes Gynecomastia

Gynecomastia results from an imbalance between estrogen and testosterone activity in the breast tissue. The most common triggers are the hormonal shifts of puberty (which often persist into adulthood), the natural hormonal changes of aging, and weight gain, which increases both fat and the conversion of testosterone to estrogen. Other contributors include anabolic steroid use, marijuana use, certain prescription medications, and some medical conditions. Because true glandular gynecomastia does not respond to weight loss, men who slim down — including those on GLP-1 medications — are often surprised to find the chest fullness remains. When that happens, surgery is the only reliable correction.

Dr. Rafizadeh's Approach

Liposuction of the fatty component. Through one or two tiny incisions, Dr. Rafizadeh uses liposuction to remove excess fat across the chest and to feather the edges so the result blends naturally into the surrounding contour. In the small subset of men whose gynecomastia is purely fatty with good skin tone, liposuction alone may be sufficient.

Direct excision of the gland. For the great majority of patients, the firm glandular disc beneath the nipple must be removed surgically. Dr. Rafizadeh excises it through a small incision placed at the lower border of the areola, where the scar is camouflaged by the natural change in skin color. A conservative amount of tissue is left behind the nipple to avoid a depressed or “saucer” deformity — a common pitfall when too much is removed.

Skin tightening when needed. Most men have enough skin elasticity for the chest to redrape on its own. After major weight loss, or in higher-grade gynecomastia with significant skin excess, skin tightening or nipple repositioning may be added. Dr. Rafizadeh assesses skin quality carefully and recommends the least invasive plan that will still produce a clean, masculine result.

The Procedure & Anesthesia

Gynecomastia surgery is performed as an outpatient procedure under local anesthesia with sedation, with general anesthesia available for patients who prefer it or for more extensive corrections. When the correction is primarily fatty and limited in extent, the liposuction portion can often be done as awake liposuction under local anesthesia. The operation typically takes one to two hours. A compression vest is applied at the end of surgery to control swelling and help the skin redrape to the new contour.

Recovery Timeline

Days 1–5A compression vest is worn continuously. Mild soreness and swelling; most men are comfortable on oral medication and return to desk work within a few days.
Weeks 1–2Light daily activity resumes. The vest continues to be worn to support the contour.
Weeks 3–4Return to the gym and most exercise, building back up gradually. Heavy chest workouts are added last.
Months 1–3Residual swelling resolves and the final flat, firm contour emerges. Areolar scars continue to fade.

Gynecomastia FAQs

What is gynecomastia and what causes it?+

Gynecomastia is enlargement of the male breast caused by excess glandular tissue, excess fat, or both. The most common triggers are the hormonal shifts of puberty and aging, weight gain, anabolic steroid or marijuana use, certain medications, and some medical conditions. Because true glandular gynecomastia is not fat, it does not improve with diet or exercise — when chest fullness persists, surgery is the reliable correction. — Dr. Rafizadeh, betterplasticsurgery.com

Can't I just lose the chest fat at the gym?+

Often no. Many men spend years training the chest only to find the fullness remains, because the problem is a firm glandular disc under the nipple rather than fat. Weight loss — including with GLP-1 medications — reduces the fatty component but leaves the gland behind. Liposuction can remove the fat, but the gland must be excised directly to achieve a flat chest. Dr. Rafizadeh determines how much of each component is present during your consultation. — Dr. Rafizadeh, betterplasticsurgery.com

Where is the scar, and will it be noticeable?+

The liposuction is done through one or two tiny incisions that fade to near-invisibility. The glandular tissue is removed through a small incision at the lower edge of the areola, where the natural change in skin color camouflages the scar. In most men the result is a chest with no obvious sign that surgery was performed. Higher-grade cases with significant skin excess may require additional incisions, which Dr. Rafizadeh will discuss in advance.

How much does gynecomastia surgery cost in New Jersey?+

Gynecomastia surgery in New Jersey generally ranges from $4,000 to $8,000, depending on whether liposuction alone or liposuction combined with glandular excision is required, the grade of gynecomastia, and the type of anesthesia. A specific, all-inclusive quote is provided after your consultation. Financing is available through Prosper Healthcare Lending. Gynecomastia is generally considered cosmetic and not covered by insurance, though documented painful or pathologic cases occasionally warrant a separate insurance discussion.

What is recovery like after male breast reduction?+

Days 1–5: A compression vest is worn continuously; most men return to desk work within a few days. Weeks 1–2: Light activity resumes while the vest supports the new contour. Weeks 3–4: Return to the gym and most exercise, with heavy chest workouts added last. Months 1–3: Swelling resolves and the final flat, firm contour emerges. Most men find recovery more comfortable than they expected.

Will the gynecomastia come back?+

The glandular tissue that is excised does not grow back, so the correction is permanent for most patients. New fullness can develop, however, with significant weight gain, anabolic steroid use, or starting a medication known to cause gynecomastia. Maintaining a stable weight and avoiding those triggers keeps the result durable for the long term.

Am I a good candidate, and is it discreet?+

Good candidates are men in good general health with persistent chest fullness that bothers them and has not resolved on its own. Younger men should ideally have stable gynecomastia (not still actively changing through puberty), and those using anabolic steroids are asked to stop beforehand. Dr. Rafizadeh and his team handle every case with full discretion — from consultation through recovery — understanding that this is a private concern for most men.

Natural, Masculine Result

I was worried about looking dented or unnatural, which I'd seen in bad results elsewhere. Dr. Rafizadeh left just the right amount of tissue and the chest looks completely natural. Couldn't be happier.
