NOSE ANATOMY – RHINOPLASTY NEW JERSEY

July 31, 2013 Dr. Farhad Rafizadeh, MD FACS Morristown, NJ

If you’re interested in getting corrective rhinoplasty surgery to improve your nose, you’re probably just starting to research the procedure online. Noses have complex internal and external architecture, and rhinoplasty is a complex type of surgery, so before you dive into the deep end, let’s get started with some basic nose anatomy.

The internal structure of the nose is made up primarily of two types of tissue: bone and cartilage. The top half of the nose is supported by bone, and the bottom half is supported by cartilage.

But to simplify how we cover nose anatomy, let’s forget about halves. It’s helpful to look at the nose as having an upper third, a middle third, and a lower third. The upper and middle thirds have the bridge, and the lower third has the base and the tip of the nose.

The upper third of the nose has the radix, the area at the very top of the nose, between the eyes. Below the radix is the bridge of the nose, also called the dorsum.

The middle third of the nose is also called the middle vault. Externally, it only features the dorsum. Internally, the middle vault is where the bone of the nose meets the cartilage. Often, the dorsum features a dorsal hump, a malformation composed of bone and cartilage. Removing this hump is an example of a typical rhinoplasty procedure.

In the lower third of the nose, the base, you have the nasal tip and the nostril area. The nasal tip culminates in two points, called the tip-defining points. The area directly above these is called the supratip. The nostril area features the alae, the flaring arches of the nostrils. The holes are called nares or nostrils. Between them is the columella, the outer skin area that separates the nostrils. The inner area that divides both sides of the nose is the septum.

Now that you have a grasp of basic nose anatomy, you’ll find it easier to understand plastic surgeons when they use some of the technical terms associated with rhinoplasty. Common Questions Patients Search About Rhinoplasty

How much does rhinoplasty cost in New Jersey?

A primary rhinoplasty in Northern New Jersey typically ranges from $9,000 to $15,000 all-in, depending on whether the case involves the bridge only, the tip only, or both, and whether septal work is needed for breathing or to straighten the nose. Dr. Rafizadeh quotes each patient an exact figure during the consultation after the computer simulation, so what's quoted is what's billed. Revision rhinoplasty runs higher because the tissue planes are scarred and the operation takes longer.

What is the recovery like after rhinoplasty?

Most Morristown rhinoplasty patients spend a week at home with an external splint, return to most non-vigorous activities in 10–14 days, and look photo-ready around the three-week mark. Internal swelling continues to soften gradually for 6–12 months, particularly at the tip. Bruising around the eyes resolves in 7–14 days for most patients and can be covered with concealer earlier.

How long does swelling last after rhinoplasty?

Visible facial swelling resolves in 2–4 weeks. Subtle bridge and tip swelling continues to soften for up to a year — the nose looks progressively refined month over month rather than reaching a final state at week four. The tip is always the slowest area because the skin there is thick and the lymphatic drainage is slowest after surgery.

Is rhinoplasty a painful surgery?

Surprisingly little. Most Northern New Jersey patients report pressure and congestion rather than sharp pain, and most need only Tylenol and a short course of a mild prescription pain reliever for the first 2–3 days. The breathing congestion (from internal swelling, not packing — Dr. Rafizadeh rarely uses nasal packing) is the more annoying part of the early recovery, not pain.

What happens 20 years after rhinoplasty?

A well-executed rhinoplasty is permanent in its structural changes, but the nose continues to age along with the rest of the face. The tip can drop subtly over decades as cartilage support weakens, and the skin envelope thins, which can make a small revision desirable at some point. The results Dr. Rafizadeh designs around — a conservatively narrowed bridge and a well-supported, reinforced tip — tend to age well precisely because the support structures are maintained or strengthened, not removed.

Will my friends and family notice I had a nose job?

When the rhinoplasty is conservative and tailored to the face, casual viewers often comment that the patient looks rested, fresher, or simply better without being able to point to a specific reason. Strangers notice nothing. Close family eventually notices — sometimes within days, sometimes only when comparing old photos at the holidays. That gap (obvious to the patient, subtle to the world) is the result Dr. Rafizadeh's North Jersey patients consistently report.

What is the hardest plastic surgery to recover from?

Rhinoplasty is one of the more technically demanding cosmetic procedures, but it is not the hardest to recover from. Tummy tuck and deep plane facelift recoveries are objectively more involved — rhinoplasty patients typically feel back to baseline in 2–3 weeks, while abdominoplasty patients need 4–6. The harder thing about rhinoplasty is patience: the final refined result takes many months to fully emerge, even though the patient feels normal long before that.
